[ITSM | How-to] Designing Custom Request Forms in AssetSonar

Why It Matters

Capturing the right data upfront eliminates the back-and-forth of vague IT requests. By building structured forms with specific question types, your team gets exactly what they need to start troubleshooting immediately.

How to Set It Up

  • Access the Builder: Navigate to Administration → Forms and open a new or existing form. Beside the form’s name and description, locate the small builder panel.
  • Structure the Layout: From here, you can structure your layout by clicking Add a comment or Add a horizontal layout (once you add an element, the option to Add a section will also appear for further organization).
  • Add Questions: To capture user data, click Add a question and select the exact answer format you need—such as a dropdown, long answer, date and time, actor, file, request type, or urgency.
  • Configure and Preview: Configure the specific response criteria, decide if the field should be marked as Mandatory, and click Preview to see exactly how it will look to the end-user before saving your changes.

Next Steps

Open an existing form, use the panel to Add a question with a Mandatory file upload requirement, and Preview the form to see how it restricts submission without an attachment.
